Job Type: Travel Contract (13 Weeks)
Shift: Day Shift (8 Hours)
Schedule: 40 Hours/Week (Primarily Monday–Friday, 8 AM – 5 PM)

Job Summary:
We are seeking a skilled and compassionate Physical Therapy Assistant (PTA) to join a Home Health team. This role involves providing patient-centered care in a home-based setting, assisting with therapy plans, and supporting recovery for patients with a variety of conditions.

Key Responsibilities:

• Assist Physical Therapists in implementing treatment plans
• Provide therapy services to patients in a home health setting
• Monitor patient progress and report updates to the care team
• Complete accurate documentation using EMR (Cerner)
• Educate patients and caregivers on exercises and recovery plans
• Ensure compliance with all clinical and safety standards

Patient Population:

• Orthopedic conditions
• Stroke recovery
• Joint and spinal injuries
• General post-acute rehabilitation cases

Requirements

• Active PTA License (state-specific)
• BLS Certification
• Minimum 2 years of experience
• At least 1 year of therapy-related experience (PT/OT/Speech)
• Experience with OASIS documentation required
