The PRN Bilingual Adherence Care Navigator 1performs outreach to patients and accepts inbound phone calls from patients and healthcare professionals to assist in removing barriers to care
Essential Duties & Responsibilities:
Makes outbound calls to patients to schedule a medication review with a Clinical Pharmacist.
Takes inbound calls from patients and other healthcare professionals pertaining to scheduling of AdhereHealth's MTM services to be provided
Possesses and demonstrates experience in positive interactions with healthcare professionals of all levels involving patient medications.
Practices discretion and care on a regular basis when working with sensitive/confidential data pertaining to the patient, department or company.
Utilizes company specific software applications to document patient call interactions
Practices excellent written, verbal, listening and interpersonal communication skills necessary to effectively interact with all levels of internal and external parties, to convey information correctly, manage difficult situations, and always represent AdhereHealth in the most favorable manner.
Leadership Duties (If Applicable)
Establish positive partnerships across business groups both inside and outside of the organization
Education & Experience:
High School Degree or GED Equivalent is required
Proficient in Microsoft Word, Excel, Outlook and Google and ability to learn new software as needed
At least (1) one-year office experience is required
Ability to work available shifts between 8:00 am - 8:00 pm CST
Must be Bilingual in one of the following: Cantonese, Mandarin, Spanish, Vietnamese or Korean
Minimum Physical Requirement:
The PRN Bilingual Adherence Care Navigator 1 will perform his or her work in a HIPAA secure work space with no background noise using standard office equipment. The following demands are representative of those that must be met by an employee to successfully perform the essential functions of this job:
Regularly lift and/or move up to 10 pounds
Bend, twist or stoop on a repetitive and continuous basis
Stand and remain on feet for extended periods of time
Operate various business instruments, including computers, printers/scanners/fax machines, requiring manual dexterity
Sit and concentrate for long periods of time to compute, examine, and interpret data
Read paperwork and computer monitors, requiring visual acuity
Skills:
Strong written and oral communication skills including ability to be able to communicate effectively with all level of management and employees
Strong time-management skills and keen eye to detail
Ability to deal with all situations in a calm and positive manner
Ability to learn new processes quickly in a fast paced environment
