POSITION TITLE: Residential Case Manager
PROGRAM/LOCATION: COMMUNITY RESIDENTIAL PROGRAM (CRP) - SAN LUIS OBISPO
MISSION: Transitions Mental Health Association is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for people with mental illness through work, housing, community and family support services.
Position Summary
This position has a client caseload split between Community Housing and Bishop Street Studios. Case Managers for the Community Residential Program supervise the general health, safety, and social rehabilitation of adult clients receiving services from San Luis Obispo Behavioral Health. They perform moderate facility maintenance, act as positive role models, promote cooperation in communal living, and uphold the program philosophy. The goal is to help adults with serious mental illness succeed in the community with structure and support. The Case Manager is responsible for crisis care, individual rehabilitation activities, day-to-day skills building, transportation to appointments, and other duties required to maintain client stability. The Case Manager will work Tuesday through Saturday providing a full range of services to CRP residents using a trauma-informed approach, with in-office work at the CRP SLO location Tuesday through Thursday and at the Bishop SLO Office Friday and Saturday.
Hours / Salary
Full-time, non-exempt, 40 hours per week. Anticipated hiring range: $24.50$25.50 per hour. Benefits include health, dental, vision, life, 403b retirement (up to 5% employer match), paid vacation/holiday time, paid sick time, and other employee perks. (Full Salary Schedule: $24.50$32.00 per hour).
Responsibilities and Duties
- Provide case management for an assigned group of clients living in Community Housing and at Bishop Streets Studios. Includes one-on-one support, symptom management, family education, advocacy for residents rights, and assistance with independent living skills (nutrition, cooking, shopping, budgeting, housekeeping, hygiene).
- Assess residents psychiatric symptoms and treatment responses. Recognize crises, provide counseling and emotional support, and educate clients about mental illness and coping strategies. Help develop problem-solving and coping skills.
- Promote trauma-informed practices and use Collaborative Communication Guidelines to facilitate clear, empathetic, person-centered communication.
- Assist clients with substance abuse education and treatment options, and identify potential precursors to substance use.
- Advocate for clients, connect them with community resources, housing opportunities, medical services, and social supports; help build healthy peer and familial networks.
- Collaborate with clients to set and pursue personal goals, review plans, and support independence.
- Assess activities of daily living and help clients develop skills to achieve independence (hygiene, shopping, cooking, budgeting, transportation, scheduling).
- Provide vocational goals support, referrals to vocational services, and job coaching as needed.
- Act as agency liaison by coordinating with the Program Manager, SLO County Behavioral Health, Housing Authority of San Luis Obispo, and other treatment agencies; coordinate referrals as needed.
- Ensure cleanliness and upkeep of residences; perform monthly facility checks; report repairs and manage furnishings and sanitary living environments.
- Complete program paperwork (client files, intake/discharge, petty cash receipts, vehicle checklists).
- Meet Medi-Cal billing and documentation goals; spend at least 50% of work time in direct client services unless otherwise directed.
- Attend regular staff meetings and complete required in-service trainings. Assume other duties as assigned by Program/Assistant Program Managers.
Job Environment & Physical Demands
Daily and frequent documentation of client services; moderate physical activity and frequent driving; may lift up to 50 lbs. Position involves work within residential environments and potentially intense situations related to confrontational or oppositional adults. Specific physical demands include seeing, hearing, speaking, stooping/bending, driving, climbing, lifting, computer use, and standing for long periods.
Minimum Requirements
- BA/BS with 6 months full-time equivalent experience with the target population, OR AA/AS or CADAC/CPRP certification with 2 years of experience, OR 4 years of experience with the target population.
- Commitment to the rights of individuals to live in a safe environment and to receive meaningful community supports.
- Reliable automobile with expected use of personal vehicle and mileage reimbursement if a company vehicle is not available.
- Strong interpersonal, collaborative, and communication skills (oral and written).
- Well-defined organizational and time management skills; ability to work independently and flexibly.
- Ability to handle intense situations with good judgment; maintain calm under pressure.
- Able to work with diverse individuals and promote equality and respect for diversity (race, gender, sexual orientation, religion, ethnicity, national origin, etc.).
- Ability to deliver culturally responsive services and understand diversity dimensions including mental health.
- Adhere to safety rules and practices to ensure maximum safety for self, colleagues, clients, and others.
- Must be at least 18 years old; possess a valid CA drivers license, adequate driving experience, reliable transportation, proof of auto insurance, and DMV record meeting TMHA requirements. May require personal vehicle use.
- Clear TB assessment and criminal record clearance by fingerprinting; able to complete First Aid and CPR training.
- Attendance is essential for client and staff interaction.
