Position Title : Case Manager
Program : Veteran's Wellness Program (VetWell)
Position Summary
The Veteran's Wellness Program engages Veterans, their families, and support persons to provide treatment and ancillary supports to those managing mental health issues, relationships concerns, co-occurring disorders, and trauma. Clients are offered services in a manner that encourages hope while promoting resiliency, growth and recovery. The VetWell team values professional and cultural humility, as well as self-reflection and personal growth/recovery, in an effort to provide high quality client care and promote wellness within the team. The Case Manager will provide the full range of trauma informed services including individualized planning, case management, integrated co-occurring drug and alcohol services, and peer mentorship. The VetWell Case Manager and clinical team work as a team to manage a caseload of 30-40 adults with serious mental illness. The Case Manager will be responsible for crisis care, case management service delivery, individual rehabilitation activities, day to day skills building, coordination of other needed supports and services, and group facilitation.
Hours/ Salary
Full-time, non-exempt position. 40 hours per week. Anticipated hiring range is $24.50-$25.50. Responsibilities and Duties:
1. Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; along with the assigned clinician and treatment team, assume responsibility for developing, , implementing, evaluating and revising treatment goals and plans; adhering to all required reporting requirements; provide one-on-one support to clients as needed; provide symptom management support; educate and support clients' families while acting in clients' best interest and advocating for the rights of the clients. Assist clients in all aspects of independent living including the development of social, vocational, and effective life management skills.
2. Assist the clinician in conducting a comprehensive assessment of mental health history, mental status, and diagnosis.
3. Provide ongoing assessment of clients' mental health symptoms and their responses to treatment. Recognize and be available to clients in psychiatric crisis or emotional distress, and assist clients in developing appropriate coping and conflict resolution skills. Provide emotional support and symptom recognition and management. Educate clients and their families about the nature of mental illness, symptoms of mental illness and coping strategies. Support clients in developing ways to manage symptoms, augment coping skills, improve interpersonal relationships, and strategize problem-solving skills.
4. Assist clients in learning more about substance use and treatment options open to them such as A.A. and other support groups. Work with clients in identifying possible precursors that could lead to alcohol or drug abuse.
5. Facilitate familiarizing clients with and encouraging the usage of community resources, support systems, services, and opportunities. Support clients in building healthy peer and familial support networks. Assist clients in developing various recreational skills and interests.
6. Discuss with clients what goals they plan to achieve for themselves and assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and strengthen and support independence.
7. Provide opportunities to allow clients to establish vocational goals and identify any job-related areas that they want to work on. Provide vocational counseling, referral to appropriate vocational services, and assist clients with job coaching and support as needed.
8. Complete program paperwork such as individual client files, evaluating and charting client progress, service plans, admission/discharge paperwork, Tri-Care billing, client security deposit receipts, time sheets, petty cash, , and/or inventory forms.
9. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
10. Attend regular staff meetings, participate in client intake meetings, and complete required hours of in-service training
11. Assume other duties and responsibilities as assigned by the Clinical Program Administrator
Job Environment:
1. Daily and frequent documentation of client services on the computer.
2. Services to be delivered to clients during a Monday through Friday workweek; schedules will be developed in collaboration with all teammates and management staff to ensure maximum support for clients.
3. Moderate, physical activity and frequent driving that will include the transportation of TMHA clients; may include moving or lifting up to 50 lbs.
4. Involves moderate risks that require safety precautions and the risk of exposure to potentially intense situations.
Requirements
Minimum Requirements
BA/BS AND 6 months of full-time experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time experience with target population, OR 4 years of full-time experience with target population. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills. Well defined organizational and time management skills. Ability to independently and flexibly. Ability to handle intense situations, display good judgment and decision-making and remain calm and clearheaded under pressure. Able to work effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level. Ability to possess and maintain good physical and mental health. Ability to work well within a team and maintain a professional, respectful attitude in the workplace. Ability to be willing to work with supervisor oversight and direction. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees. Conditions of employment include: Must be at least 18 years of age; possess a valid CA Driver's License, at least 2 years driving experience and DMV driving record that meets TMHA's current vehicle insurance requirements. May be required to utilize reliable personal vehicle and auto insurance. Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
Preferred Experience:
Experience as a Veteran or family member of a Veteran.
