Patient Care Manager Opportunity Description



Our client is a well-established Home Health organization. They have a current opening for a Patient Care Manager for their Home Health location in Salinas, CA. Medicare Home Health experience as an RN is required. Supervisory experience strongly preferred but may consider a strong case manager / field nurse ready to step into a management role. Patient Care ManagerJob Requirements

• Medicare Home Health experience as a Registered Nurse.


• Excellent understanding of state/federal regulations, OASIS, and PDGM.


• Supervisory experience strongly preferred.

Patient Care Manager Job Responsibilities

• Manage clinical team.


• Ensure compliance with all state / federal regulations.


• Train and mentor nurses.
